Informing us of absence (sickness)

Complete the form below to alert us if your child is not coming to school due to sickness, hospital or doctors appointment, dentist or other.

Sickness absence or appointments reporting can no longer be reported on our Rushmere Hall website.

These must now be reported via the MCAS app. Please follow the attached guidance.

 

If your child wont be having lunch...

Please make sure you cancel this directly with The Pantry. You can do this through your account OR by phoning 020 8813 7040. If you do not cancel you will still be charged for a school dinner.

Informing us of absence (holidays, weddings and other absence from school)

If you need to inform us of an upcoming absence e.g. a holiday, a wedding etc. 

There is no entitlement in law for pupils to take time off during the term to go on holiday.

INFORMING US OF ABSENCE FORM 

The Education (Pupil Registration) (England) Regulations 2006 were amended in September 2013. All references to family holidays and extended leave have been removed. The amendments specify that Head Teachers may not grant any leave of absence during term time unless there are “exceptional circumstances” and they no longer have the discretion to authorise up to ten days of absence each academic year.

Please read the FAQs before requesting medical evidence, paying for x-rays and other documents to proof a need for absence.

Informing us of absence 
It is your responsibility to inform us of your child's absence from school. 

Complete the form to let us know of any absence for your child/children. 

Please ensure you are truthful and honest about your absence as we will complete checks. 

We will track absences carefully and where necessary the school will refer cases to the Education Welfare Services.

Authorised absences
Authorised absences may be planned or unplanned; they are mornings, afternoons or whole days away from school for an unavoidable reason like illness (although parents/carers may be asked to provide medical evidence for an absence before this can be authorised), medical or dental appointments which, unavoidably fall in school time, emergencies or other unavoidable causes. The pupil’s parent/carer must also apply for other types of term-time absence as far in advance as possible of the requested absence.

Medical appointments 
We encourage parents/carers to make medical and dental appointments out of school hours where possible. Where this is not possible, the pupil should be out of school for the minimum amount of time necessary and return afterwards, or leave school slightly early to attend the appointment. The school may request proof of appointments. 

Unauthorised absences 
Unauthorised absence includes:

  • parents/carers keeping children off school unnecessarily e.g. because they had a late night or for non-infectious illnesses or an injury that would not affect their ability to learn.
  • absences which have never been properly explained.
  • children who arrive at school too late to get a mark on the attendance register.
  • shopping trips.
  • looking after other children or children accompanying siblings or parents to medical appointments.
  • their own or family birthdays.
  • holidays taken during term time without leave - 3 days unauthorised absence will lead to a penalty notice being triggered by the Local Authority.
  • day trips.
  • staying home due to weather conditions (e.g. snow) unless directed to do so by the school.
  • other leave of absence in term time which has not been agreed.

Green Cards

GREEN CARD FORM

If you have already given permission for your child to leave school by themselves, you do not have to complete this form again.

Please provide us with confirmation if you are allowing your child to leave school by themselves – ideal for parents who are allowing their children to walk home from school by themselves or to meet them at an agreed location e.g. KS1 playground.

The default arrangement is that children are NOT allowed to leave school by themselves.

Complete and return the form to your child’s class teacher. Copies of this form are available from reception.

 

Please NOTE this option is not permitted for children in Year 3 until the Summer Term. 

Children will be provided with a GREEN card to show they have permission to leave school by themselves at the end of the school day. 
Please ensure your child is clear about the arrangements you have for them and what to do if things change e.g. what to do if they get home and you are not there OR what to do if you are late getting to your meeting point. Children should know that they can return to school if they are unsure and speak to someone in the school office. 

Photo and video permissions

Complete this form only if you:

  • are new to the school (and didn't complete the form in your welcome pack) 
  • want to change/clarify your preferences

Occasionally, we may take photographs of the children/young people at our school. We may use these images in our schools prospectus or in other printed publications that we produce, as well as on our website or on project display boards at our school. We may also make video or webcam recordings for school-to-school conferences, monitoring or other educational use.

From time to time, our school may be visited by the media who will take photographs, film footage or carry out radio interviews. Pupils will often appear in these images, which may appear in local or national newspapers, or on televised news programmes.  Photos for the media and other publicity purposes may also be taken at events where our school is taking part.

Conditions of use

  • Once completed this form is valid for the period of time your child attends this school, plus one year after they leave, to enable us to publicise the work of final year pupils. The consent will automatically expire after this time. 
  • If we use photographs of individual pupils or small groups of pupils, we will avoid using the full name (which means first name and surname) of that child in the accompanying text or photo caption. The same rule will be applied to video content. Please note that the media are likely to use first names and surnames in their publications/broadcasts.
  • We will not include personal e-mail or postal addresses, or telephone or fax numbers on video, on our website, in our school prospectus or in other printed publications.
  • We may use group or class photographs or footage with very general labels, such as “a science lesson” or “making Christmas decorations”.
  • We will only use images of pupils who are suitably dressed, to reduce the risk of such images being used inappropriately.
  • You can request at any time for the consent you have given to be removed. Please complete this form again showing your new preference. The newest form received by the school will stipulate your current preferences. The school will try to remove all content uploaded before the change of consent.
